when the approvals are created from WF in change, the approvals tab in CHG shows up as "not yet requeted" next to the approvers name instead of "requested". What could be the possible reason for this?

Are you sure that you don't have any onload client script or display business rule setting this on the form ?

Can you please try clicking on show workflow and see whether it reached the Approval stage or something before coming to Approval is holding up the flow??

It has gone to the approval coordinator. But all the approvers show up as not yet requested and no emails have been sent out.

In that case try to see the template that triggers the notification, if any recent updates has done on template which make approvals not triggering..
There is no any record in logs also that email has sent??

Hi Ravali,
Before It has gone to the approval coordinator, you need to use Run Script activity and need to update there as
current.approval = 'Requested';
