Hi, do you see any errors in your browser console?
Have you added\updated any client-side scripts\functions that would load for incident and not problem?
if yes first step might be to revert or disable these changes.

Hy Patron, thanks for checking I have reverted changes which was added by me still am facing same issue.

Hello,
As Tony wrote, this is most likely because a Client Script or UI Action has code in it that doesn't work.
If you open the console in your browser when looking at a record, you should find an error somewhere indicating what script is failing you.
For Firefox, you get the console with CTRL + SHIFT + J
Adding a screenshot from the console for this page, as an example of what you might find.
