Playbook Experience-Risk Indetification creating two sys_pd_context records on reinitiating workflow

I am trying to reinitiate playbook workflow on last activity which has a reject button. On Data Broker server script i am calling:

new sn_risk_advanced.RiskIdentificationUtils().reinitiateRiskIdentificationWorkflow(riskIdentificationObject) and pasing the current record. It is recreating context correctly but it is doing it twice

How can this be resolved.

Can you share the data broker server script screenshot? And try to verify where data resource is calling, if you could also share the screenshot.
