Redirect user to pre-populated service catalog item using UI action

Hello,

 

From a separate form (of course), I am being requested to redirect the user to a service catalog item and have several values pre-populated. I'm being able to redirect the user, however, I'm having trouble pre-populating the values. The pre-populated values should come from the original form where the UI action is being clicked from.

 

Please help!!!

What UI is the user in when they click the UI action and which UI are they being sent to via the redirect to the Catalog Item form?
