Requests are getting auto approved while Adding additional approvers to the list on RITM form

Hi all

Recently we have encountered issues while adding additional approvers to the list on RITM form. And after adding the approvers to the list, the request is getting auto approved. Has anyone faced this issue before?

The issue is solved for us after the Utah Hotfix Patch 7.
