Hi Community,
I have a requirement to display the Risk Assessment questionnaire directly inside a Change Request form section (tab) instead of accessing it through the Related Link.
Is this supported OOTB?
Can the Assessment questionnaire be embedded directly inside a Change Request form section?
If not, what is the recommended approach?
Any guidance or experience with a similar requirement would be appreciated.

No that's not recommend and increased the technical debt. Use the way it is given buddy. You can add condition minimum one risk assessment should be completed.

Displaying the native Assessment Designer questionnaire directly inside a Change Request form section or tab instead of using the related link is not supported OOB in ServiceNow.
Customization here is not recommended for following factors:
- Customizing core assessment display logic means your solution will likely break or require heavy manual refactoring during major platform upgrades
- OOTB risk assessment questions are dynamic metric categories rendered via UI Macros/UI Pages, not standard table columns. Embedding them requires complex custom UI Macros, Jelly/Client scripts, or duplicating data into custom fields.
- Loading an entire interactive assessment engine synchronously inside a standard ticket form can degrade form load times.
- Any updates to questions, weights, or metric categories in the Assessment Designer will not natively map or scale cleanly inside a hardcoded form section.
