RITM mandatory fields are losing its values once form submitted

Hi All,
I'm facing a strange issue in my production instance.
We do have a Catalog Item - 'DB Access Request' form. It has few mandatory fields. In some cases, submitted form does not have values on the mandatory fields.
Due to this, Approvers are not able to approve the request. This is not happening every time, but in rare cases. Even we have tried to reproduce the issue but no luck.
Also one more thing is, only the text fields are missing its value & not the lookup fields.

Post submission (when I tried, the submitted form fields aren't blank. Just for a reference am attaching the below screen),
Can any one help me on this. I'm struggling with this issue for more than 2 days.

Please check any On load client running,
Other wise you should raise the HI Ticket

Hi
Iam not sure how are you populating the values on the fields like manually or by any onload or onchange scripts
If it is related some thing with onLoad / onChange Scripts , Then Please ensure you check the "Applies to Requested Item " As well In the Client Scripts or UI Polices
