Service Catalog, Catalog Item to search for previous incidents

We have a requirement to create a Catalog Item that will create a specific category of incident.  However, when the user wants to write the short description for the incident, we want the variable to do a lookup for existing incidents so they can check if the specific incident they are entering has already been reported.  Is there a way to display these incidents below the Short Description variable in question?

What to you meand by 'we want the variable to do a lookup for existing incidents so they can check if the specific incident they are entering has already been reported'.Is it mean that, you want to match the short description to the incident table which has some key words or match at least three words on existing incident table short description ! or how you want it.

If the incident has been reported found based on short description..what would be the action ..you want to create child incident or you want a invalid or prevent submitting the catalogue items.what you want to do in action?

Secondly, How you want to build the solution ..is it on client-based script or server-side script or Both or any preference?
