Service Catalog - If radio button checked == true then display textbox

First day on the job working with Service Now so I am needing a tad extra guidance.

 

I am in the Service Catalog learning my way around and creating some variables. In one of my questions, I am giving the user a multiple choice question they answer with radio buttons. I want to display a text entry field only if they select a certain radio button and then make it go away if they un-check it. I have the Wiki open and Google etc.. I am not even sure where the JS would go for something like this. Any direction would be appreciated.

As user friendly as ServiceNow is, I assumed something like this may exist.
