Hi All,
I'm having issues with creating new delegates from the Service Portal, when I click 'New' at the top of the list then the following page is blank (while impersonated normal non-admin user) and I am unable to fill out the form and save since it is just a blank page. When I use the add 'New' as admin, I get small table element with 'Delegate record details' but nothing to fill out. When trying to edit current delegation then I get some table elements but there aren't any details nor ability to edit, this is the same behavior for admin and non-admin users. I am able to accomplish this from the Portal (for both admin and non-admin normal user) when I navigate to the Profile I see the Delegates list and when I click the 'New' button, it takes me to the form sys_user_delegate.do so it works as expected. In Portal I am able to create/edit/delete delegate records from the Profile, from Service Portal I am able to view current delegates but I am unable to create new ones or edit current delegates (delete option is not available in SP but that is expected).
I first implemented this in my PDI and all is working as expected, Service Portal 'My Delegates seeKB0759035' takes me to the list then I am able to add new delegates and edit records as expected. The same is not working in my Dev environment but the ability to add delegates via Portal > Profile is functional.
How do I make the delegate form work in Service Portal?

I ended up using 'ServicePortal delegate widget' by Fredrik Larsson, added to the new page I created and then made the page accessible through the header link.
