Submitting an Survey Anonymously

Is it possible for a user to choose if he wants to submit the survey anonymously or with his user id? Client has an a requirement to give this option to the user. When I checked, it is OOB widget. I am trying to figure out how to do it without editing the widget - Surveys and Assessments. Any suggestions?

No. If you want it to be anonymized, it's done upfront. You could do it the other way around: make it an anonymous survey with the final question being: 'if you want to, you can leave your name/email for follow up'.
