Subscribe to Notifications on Major Incident

Hi

I want user to be able through a Catalog Item to subscribe to notifications when a Major Incident occurs.

The user should be able to select channel like Email, Slack, MS Teams and SMS.

Any suggestions how to achieve this in an "easy" and sustainable way.

Add a multi-select field for users to choose notification channels (e.g., checkboxes).

Optionally capture contact info (Slack username, phone number, Teams ID) if not already stored in the user record.
