Survey not sending

I created a survey for incident.   Added a condition that when the incident is closed to send a survey.   The notification is not sending.   I dont understand why the OOB notification 'Incident Survey' with a trigger on task.send_survey is not sending. I can see that the assessment is generating just no notification. using geneva

So the OOB conditions on the notification 'Survey User Invite' does not seem to be correct.   I went into the assessment and validated all the conditions were meet, so i decided to change the assigned to and see if the survey would fire.   And Eureka it did.   So i am wondering why the OOB conditions are not correct to send out the assessment notification?

Hello Sigmachiuta:

In your trigger conditions, can you add 'State is 'Closed' or State is 'Resolved' and try again.

actually the state is the state of the survey, I changed the OOB condition to assigned to is not empty and the notifications started to send.

I am glad it worked.   I thought, you are creating survey for the incident, in that case,   OOB survey sends notifications when you configure like I said.
