Survey not triggering for catalog item

I have created new Survey for a particular catalog item, but it is not triggering and also when i try to View Survey URL, it showing an empty page.

Hi @sivaprasad0725 ,

 

i would advise you to go through the trigger conditions to be sure that all condition are meet. Also, why have you marked it to trigger randomly?
