Two REQs with same number but different sys_id is generated for a catalog item

Hi Team,

There are two REQs generated for a catalog item with same number but different sys_ids. Can someone please help me how to debug it. Like how does a REQ generate for a catalog item, what's the first point for me to look at?

The process is:

REQ - RITM - SCTask

It’s very rare for two REQs to have the same number.

This applies to incidents, but the same logic can work for REQs.
