UI Action reverted to previous data without a manual revert

 I did not do a manual revert but a specific UI Action's script were reverted to the previous version. I would like to know what caused this. The instance version is Tokyo. If anyone knows, please let me know.

Was an upgrade performed and someone rolled back a customization? Have you looked at the versions related list and checked who shows as the user that updated the reverted action?
