UI Policy Needed for a Survey

Team,

Is there anyway I can set a UI Policy for a Survey, when (No) is seleted, and additional comment box will appear to enter information of why you selected no.

I have tried the SN WIKI and tried in my DEV instance and I had not luck thus far.

Surveys don't have access to client scripts or UI Policy. They were created before either existed and were never enhanced with them.

Survey Wizards have support for both, though they're somewhat more difficult to set up.

I have activated survey wizard plugin now but facing an issue creating UI policy for the exising survey (which is in survey> masters)

What is the alternate solution for Surveys to show questions based on answers of the previous question.
