** URGENT ** MS Teams - Actions on provider notifications not working (not updating records)

Hello, 

Can someone please guide me why the actions that i have created for provider notifications don't update or take any action on the records as expected? I just see 3 dots after clicking the button and then the conversation ends abruptly. I have created the action and added it in the 'link actions to content'

I'm not even getting any acknowledgment statements back. Nothing happens at all.
