User Impersonation is not working properly

I am facing an issue with user impersonation. Whenever i click on that it is displaying(Please check the attachment), but it is not working. Please help me out of this issue. Thanks in advance.

I have hopped to your instance to debug the issue. The issue is resolved now and it is working fine . Please check

I see you created a custom UI page impersonate_dialog which was not allowing to render the dialog properly. I have just changed the name from impersonate_dialog to impersonate instead of deleting.
