Initially I thought this issue was due to individual workflows associated with particular items that had been requested, but I couldn't see any issues within the workflows. Then I discovered this appears to be across all RITM's that have reached a stage of 'request_approved'. What's interesting is that there are SOME RITM's that are 'request_approved' and do display the Stage progress and I'm unsure of why that may be.
We have had a Hi ticket open for this and they had recommended upgrading to Kingston to resolve the issue, but that didn't resolve it and they have since said the issue appears to be on our end, but weren't sure why/how it was happening.
Here is what the issue looks like across all RITM's.
A few things to note:
- The RITMs that do show a working Stage progress, all appear to be before a particular date(not 100% though). I haven't been able to pinpoint the date cutoff though, but it seems apparent over many RITMs.
- I have checked Default Stages, Stage Sets, Workflows, etc.. and have been unable to remedy this issue.
- SNOW Support had recommended upgrading to Kingston to resolve, but this didn't resolve the issue.
- I initially thought it could be per RITM workflow, but then I discovered it was happening across basically all RITM Item types.
I'm really looking for any help/advice on this as this issue has been happening for close to a year and we haven't been able to pinpoint and resolve this issue, even with the assistance of SNOW Support.

Please look at the Default Stages (by table) table (stage_set_table). Make sure there are no records in the table that are causing the the default stages to error. The records in that table requires two required fields, table and set fields. If one of those are incorrect, it could cause the stages to appear incorrectly.
