Why workflow Catalog tasks are not created when workflow run?

I have an issue on my instance where workflows are getting auto completed after approvals are set, can somebody help?

i.e:

I raised a RITM where should create a task to my IT team but after manager approves request workflow seems that goes into all activities it looks like all of them ran successfully,  RITM is closed complete but the catalog task never was created.

Hello Sanjiv Actually Catalog Tasks are not even created on the RITM... There are 0 task on my ticket and workflow is completed... That is the issue that I am seeing globally on the instance, not just one single workflow. Thanks, Lupita

Can you post the screenshot of the task activity in your workflow?

I want to see the script. Is this workflow running on Request or Requested item?

Hello

 

There is no script on the task, I am just assigning to my Service Desk Group, and that is all. Actually any workflow on the entire instance is working now...  

Do you see any error in the system log when you run the workflow?

No, any errors are logged on System Logs, also checked most of the scripts and I am not able to find anything that can break workflows in a globally way.
