could you help me with the steps on how to create UI Policy for the tak below.
Show additional fields for 'Network' category incidents:
- Objective: Provide additional fields for capturing relevant information specific to network-related incidents.
- Condition: Incident Category is 'Network'.
- Action: Show additional fields such as 'Device Type', 'IP Address', and 'Port Number'.
In UI action policy - I am able to add only 1 field type. How to add one or more field names(like IP address, device type, in order to show up in the form).

HI @LikithaK ,
I guess Port Number and IP Address are custom fields, I done with additional assigne and Assigned to field as I show in below image. You can see the result
1. When Category is not Network
additional assigne and Assigned to are not visible
2. When Category is Network
additional assigne and Assigned to are visible
Here the UI Policy Action for reference

Thank you for your effort and time for the reply.
But,my query is to get 2 fields for example port number and IP address fields should be appear in the Form, like service and configuration item in the Form, when I select category as Network.
