Adding Custom Application Attributes in APM

Bringing up APM for the first time and was looking for guidance on:

1.  How to add custom attributes for Business Applications

2.  How to modify forms\UI to allow for proper maintenance of new attributes.

I should note that we are rolling out APM in advance of CMDB due to resource availability.
