Design QUESTIONNAIRES on the fly for set of users of type Yes/No, Attachment to collect evidence?

Hi ServiceNow Experts,

Need your valuable inputs on the below requirement and its feasibility in ServiceNow:

Requirement -
Client wants to design questionnaires which can vary from one audience to another and doesn't follow any Standard Template. The Questionnaire need to be sent out to a group of users to collect responses in the form of Choices/Attachments/Yes-No, etc. 

Any different ways we can solution it in ServiceNow to provide Client a way to prepare Questions and send them to a certain audience to fill it? Please share.

ServiceNow provides surveys application which lets to create questionnaires, send them to users, and collect their responses. Surveys can be assigned to one or more users and used across different records in the system.

If you wanted different approach from this, you can point it out as maybe I’ve misunderstood your requirement/question.

Thanks for your thoughts.

Assessments/Surveys is the last option we would think of. As in our case; the Client wants to handle or create their questionnaire as per choice while working on Evidence Collection as a part of Audit Process to be specific. 

I would like to understand if we have something other than Surveys OR any way we can build instead of opening up the Assessment/Survey level global access to the Clients for configuration.

Hope I was able to highlight the challenge well

Smart Assessment might be the thing you are looking for.

They brought a thing called "collab" with this (be sure the plugin "Smar Assessment Collaboration" is active), and you can put togehter your assessments as you like, with multiple questionaires. So it should be possible to build one assessment for multiple respondents.
