How to Insert Table of content in Plan Document in BCM

I have created few Template Script to populate my data. Now, I want that data to be inserted in Clickable form in  Table of Content.

 

If any one can help, that would be really helpful.

You should leverage the leverage the table of contents feature in the document template.
