1. On the Issue Form, I want the Attestation number to be appended to the 'Name' field on the Issue Form page. 2. From the Attestation record, I want to fetch the user's response and include it in the 'Description' field on the Issue Form page. 3. The priority of the Issue needs to be set based on the user's response captured in the Assessment Instance. I wanted to know one more thing that is it possible to create an report/dashboard for the responses we receive for all the assessments. kind of a single page where we can present this resposes.
