Inherent risk questionnaire template not attach automatically when I select start onboarding in the due diligence request. It is throwing me below error

Could you please share a screenshot for better understanding?
As per my understanding, after clicking "Start Onboarding":
You’ll need to go to "Inherent Risk Assessment"
Then click on "Questions"
After that, select "Add" and choose the type of question you'd like to include for the user
Please refer screen shot, please let me know if I'm wrong and you're looking for something else.

@prerna_sh We had an IRQ Template that automatically attached once we click on start onboarding. Recently we deactivated that and created a new one. From then we are getting the above error.

Correct am getting this error. If I activate the last IRQ, am not getting the error and questionnaire is automatically attached. Couldn't find the configuration where i need to make changes.
