Smart Assessments for Health and Safety Inspections

Required Plugins:

Health and Safety Risk Management v8.1.2

Migrating a Survey to Smart Assessments

  • Log in as a user with the Survey administrator [survey_admin] and Smart Assessment Admin [sn_smart.assessment_admin] roles
  • Select All
  • Search for Template Migrations

  • Select New

  • Select the Source metric type

Tip: This will be your existing Inspection Survey

  • Select the Purpose

Tip: Out of the box, Health and Safety is delivered as a purpose. Additional purpose categories can be configured.

  • Select Migrate
  • Migration status will change from In progress to Complete

Refresh the page

  • Verify the section migrations. If the question has been migrated completed will appear in the row. If the question is not migrated, then Not supported will appear in the row.

Note:  Note all survey metric types are supported. Please refer to the list below for unsupported metric types.

Metric Types Not Supported for Migration:

  • Custom
  • Duration
  • Image Scale
  • Percentage
  • Rankings
  • Ratings

 

Once the migration is completed, navigate to the Assessment Workspace

  • Select Workspaces and Assessment Workspace

  • Open the migrated survey

  • Select General then Details

Note: The template name will be the same name as the survey. The template name can stay the same or be updated.

  • Select Assessment as Inspection
  • Select Settings

  • Input sn_hs_rm_safety_inspection_writer under Assessment Reader Role
  • Save

Navigate to Questions

From this view, verify and update the questions. Additional questions can be added.

Creating a New Inspection Template from Assessment Workspace

  • Log in as a user with Smart Assessment Admin [sn_smart.assessment_admin] role
  • Select Workspaces and Assessment Workspace
  • Select New Template

  • Enter the following:
    • Template Name: choose any name
    • Description: choose any description
    • Purpose: Health and Safety
    • Assessment Target: Inspections
  • Select Create

  • Select Settings

  • Input sn_hs_rm_safety_inspection_writer under Assessment Reader Role
  • Save

Navigate to Questions

 

From here, build your sections and questions for your Inspection template.

Creating an Inspection using a Smart Assessment Template from Workspace

  • Under Assessment Type: Smart Assessment
  • Choose the desired Smart Assessment Template

Note: For each inspection or inspection schedule, Inspection Managers can choose if the Inspection will use a Smart Assessment or Survey.

  • Populate the rest of the inspection
  • Save

Creating an Inspection using a Smart Assessment Template from Agent Mobile

  • Select Inspections

  • From the list view, employees can see assigned Inspections. These inspections are either assigned to them or to the group they are in.
  • In the top right, select
  • Select Create New Inspection

  • Populate the inspection form
    • Assessment Type: Smart Assessment
    • Smart Assessment: Choose Any
    • Populate any additional fields as required
  • Submit

Note:  For inspection re-assignment to another user in the same group, make sure to populate the Assignment Group

 

From the list , open the inspection record.

  • Take Inspection Questionnaire

The inspection will open and the assigned user can then complete the questions.

Partial Save

When Partial Save is selected, any questions that have been answered will sync. Safety Inspection Managers will be able to see those answers from within the Health and Safety Workspace. If the inspection is re-assigned, the new assignee will see the previously answered questions.

 

Actions List

When the Actions List is selected, users taking the Inspection can create a Health and Safety Action from that question.

The list navigation allows for the user to add one or more than one action per question. The list view will filter to the actions per question.

From the Inspection record, the user will be able to see all created Action Records. If an action record is created, then an associated finding record will also generate. Additional actions and findings can be created from the list view.

Inspection Re-Assignment

Users in the same group can re-assign inspections to themselves. From the list view, open an inspection not assigned to the user you are logged in as. At the bottom of the Inspection screen, select Assign to me. This will re-assign the Inspection.

Note: In the Activity stream on Agent Mobile and the Health and Safety workspace, you can view the audit history of who has been re-assigned from the Inspection.

 

Offline Inspections

While offline, assigned Inspections can be taken. In the January 2026 release, while Offline Actions and Partial Save cannot be accessed. Please refer to the matrix below for reference.
