Employee Form Error (One or more questions were not found on the employee form. Were the questions deleted from the associated survey?)

I am having an issue with the Employee Form-powered HR Task Type to Collect Employee Input.

I have recreated the below in a PDI.... I am trying to collect one String field and map it to HR Profile.  The task and form are created and assigned as expected, and the Task closes once the form is submitted, but it does not save the value to the HR Profile field to which it's mapped, and while I'm seeing an error, the error description doesn't seem to be the issue at hand.  Below are some pertinent screenshots... Any advice would be appreciated!

Mapping doesn't appear to have any broken links or references, making it seem like the Question is mapped correctly

There are some issues with field mapping not working, but they should have been resolved in Rome.  I haven't taken the time to try to recreate this personally, but verify users have access to update their Place of birth by confirming the write ACL on sn_hr_core_profile table and the place_of_birth field.

@kurtbell have you checked any RCA records which may have spawned from this action? I have not seen this error before on the Employee Form, but I would also ask if you added the field to the white list of the HR Properties of editable fields. Typically this would give a write error but I think something else is stopping the action prior to this.
