Please find below some of the high level tasks.

1. HR data migration to scoped tables. (Both OOTB and custom tables)

2. HR roles migration to scoped tables. (Both OOTB and custom HR roles)

3. Ensure all client/server scrips work similar way on new scoped tables.

4. Customize HR portal to work with scoped tables.

5. Assess/compare legacy catalog items with scoped HR catalog and decide whether customize/remove.

In addition to the items that Kushal provided, we have a number of resources available to help educate customers on the migration to the scoped app.

  1. HR Migration Tool Walkthrough Video
  2. HR Scoped Application FAQ
  3. HR Migration Plugin documentation
  4. Migrating from the ServiceNow Non-Scoped App to the Scoped HR App - Ask the Expert! webinar
  5. HR Centers of Excellence Overview

One additional recommendation is to not view the migration as simply a "lift and shift" migration.  We hear from a lot of customers that a lot of functionality that they previously custom built is now available as delivered functionality within the product.  I'd recommend a comparison of the delivered application to determine what truly needs to be added during the migration.

Also, do not overlook the new COE and HR Services model because a majority of new functionality is based on that structure.  The COE video referenced as #5 helps explain this further.
