How to add a Service Portal widget to UI 16 Form

Hi Community,
I'd like to implement a Service Portal widget into a native UI form (HR Case, for example). I'm stuck because even though I created the formatted and added it to the form, I have no idea how the UI Macro should look like.
This is how it looks in the Service Portal and everything works and loads properly:
In the end, I want to get the calculate widget into the form as shown here (last picture):
Any help or tips on this is greatly appreciated. I attempted to follow along but was unsuccessful at producing the same result as this author:

What did you do to try it? Are you getting any errors in the console?
Does anything render in the HTML?
