How to get Create New Case form for Custom CoE in HR Agent Workspace

We created a custom CoE by extending HR Case [sn_hr_core_case]. We have matched OOB CoE configurations in all areas (that I can think of): Dictionary Attributes, Inbound Actions, Notifications, Layouts/Views, SLAs, Assignment/Matching Rules, ACLs. The CoE is active and there is no custom security. We have a basic HR Service and General Question Record Producer for the CoE.
We have only one problem. New from HR Agent Workspace takes you to a form instead of Create New Case page. I can't find what I'm missing to get the redirect done correctly.
In HR Agent Workspace I have lists for each CoE and all lists have the New button.
When I click "New" on Disability Cases (custom CoE), I am redirected to a new form instead of the Case Creation page
All other OOB CoE redirect correctly as seen below

Do you have Workspace Experience > Workspace View Rules defined for your new custom COE table?

I do - I mirrored the View Rules as defined in OOB CoE for the following:
System UI > View Rules and
Workspace Experience > Forms > Workspace View Rules
Interesting though is that I recreated the issue in a developer instance with the same results and in that instance there are no Workspace View Rules for any of the CoEs and still my custom CoE is the only one which doesn't redirect to the default case creation form. The customer instance is Quebec and my PDI is Rome.
