How to restrict Ui policy functionality to HR case form only?

Hi All,
Actually i have a requirement that , Actually i have added some fields to the case form and as well to the case creation also, basically i need to make some fields mandatory on the HR case form only ,so i have written one ui policy , but due to ui policy the fields are mandatory on the case form and as well case creation page also, but i need only on case form?
Please help me to achieve this

Hello @Ramu6
For Case Creation Page, it will show the record producer catalog form, If you want to show the mandatory fields only on the case form of native UI, Just write the general UI policy.
If you write Catalog UI Policy it will reflect on case creation page.

can you specify the tables where you have added these fields? screenshots will be helpful.
