In servicenow, how to pop up feedback menu on header in service portal with some questions

OOTB you don't have option to configure your requirement.

The only way is using a customized approach. what i would do is use a modal which is a pop-up and i can have couple of question built in it on the portal.

Hi @Nithya Devi , one option would be to use the Employee Centre Experience Feedback widget.

Experience and service feedback (servicenow.com)

You can see an example of this on the right of the viewing page for this community...

When selected it will pop out with configurable feedback options...
