Lifecycle Event not getting triggerred

I created a new HR Service and linked the New Hire onboarding (demo) record producer to it.
Then I created new lifecycle event which has trigger as immediate and after I submitted the record producer form, the case was created but the Lifecycle event was not triggered it seems and the tasks were not created.
Event type was onboarding.
As I am not using OOTB HR service for onboarding , I tried to change trigger of LE to HR service & defined my new HR service but still the LE didnt trigger and no task were created,
Can someone suggest what could be the issue here

CAn you share some screenshots of your all the LE service configurations and LE activity sets.

Hi @Saksham23 ,
Your HR service Configuration is not complete, that's why you are getting an issue!!
You must use the "Template" field, "case options", etc, use the below example for your reference:
