Lifecycle Event not getting triggerred

I created a new HR Service and linked the New Hire onboarding (demo) record producer to it.
Then I created new lifecycle event which has trigger as immediate and after I submitted the record producer form, the case was created but the Lifecycle event was not triggered it seems and the tasks were not created.
Event type was onboarding.

As I am not using OOTB HR service for onboarding , I tried to change trigger of LE to HR service & defined my new HR service but still the LE didnt trigger and no task were created,

Can someone suggest what could be the issue here

I think you need to create an Activity record (see below) for your HR Service and then add the new created activity record to the activity set., When your activity set is triggered. your service shall be excuted.
