Rearding creation of approval tasks

Hello,

I have a flow named X--> when the request is submitted through catalog item according to the flow it first creates approval task after it is approved then it is assigned to other group.This is working as expected.
So when i am creating the other request and when the request is assigned to fulfiller and when fulfiller tries to transfer the request to practice area which is same as X the new request is getting created but the approval tasks are not getting created, its directly going to the step after approval tasks. Is there anything i missed out.Any suggestions would be helpful.

"new request is getting created but the approval tasks are not getting created" , in the flow execution log - for second request check whether approval group is getting changed or it is same.

Skipping approval can happen when group having no member, inactive member.
