Flow error

I have submitted request through the form for adding group members to the group. there is 2 approval for this for one is user manager approval anather one is group manager approval so. after 2 approvals also form showing still waiting for approval. I have checked the flow In the flow showing error (error occurred while inserting record:null). I have checked user record every thing is good but why the user not adding that group. I have attached screenshot shot please review and help me on this.

Hi @manikantade,
First of all, please do not share company and nor the personal details (the first picture of your second message) - please get it anonymised or remove it.
Secondly, what debugging methods have you tested?
Was the added user always had populated manager?
The error message mention coalesce fields set to user and group, is that desired behaviour?
Could you please share the flow itself? from the test execution it is not understandable much.. and do not forget to hide the personal details.

is that user already part of that group?
Did you check that and flow might be trying to add it again?
