Need to create check box field automatically, when an incident ticket is raised

For P3, P4, P5 incidents create a field proposed problem, and it should be a check box type.

For a P1, P2 incidents a problem ticket should be generated automatically. For P3,P4,P5 incidents create a field Proposed Problem and the field should be of check box type.
