when i create new problem ticket or open problem ticket from list view I am not getting full page

when i create new problem ticket or open problem ticket from List View, i am not able to see the full page of problem ticket why i don't know. can anyone please check this i have attached Screen Shot also.

I have attached the Screen Shot please once check it. I am not getting the full page of problem ticket means i am not able to see the fields like Caller, Priority, impact, urgency , short description, description, etc...

The reason is that you're in the Notes view. Please change the view to the default CI view and test again.
