Oracle clustor/server server discovery updating wrong record

Server discovery is updating the wrong record in the Oracle Cluster 

Two servers are linked by Oracle Cluster with different IP but when discovery run its overwrites the records and changing its name from ex. 34 to 39. I have checked IRE its fine.
