Trigger ID is missing on Survey assessment instances

For Few assessment records, Trigger ID is missing.

Can someone help me with possible reasons?

We haven't done any customization on OOB scripts.

Could you open any of above record & look if the incident number shows up in form view.

1) Pls compare both the notifications for RITM & INC & check if there is any difference


2) Check if notification has been hard coded

The Trigger ID will be referred in the Survey Instance only if the Survey instances were created through Trigger Conditions. Did you 'Enable Public Access' for this Survey and stare the URL with users? Or did you copy the Survey URL by clicking on 'View Survey URL' and share with users? In either of these cases, or if Survey instance is created by any other ways without using Trigger Conditions, the Trigger ID wont be populated.

Could confirm what would be the best possible way to check which survey is associated to which record. Since, trigger ID has something that relates but in our case it is blank so of no use. What would be the alternative for associating record & survey.

Except the Trigger ID field, there is no possibility to check which Survey instance is associated with which incident record.
