Trigger ID is missing on Survey assessment instances

For Few assessment records, Trigger ID is missing.

Can someone help me with possible reasons?

We haven't done any customization on OOB scripts.

Can you suggest any other possible solution to relate record with survey other than using Legacy survey.

Hi,

I'm also finding this issue when we Create an HR Task of 'Acknowledgement Type=Take Survey' - this creates a new assessment instance record but does NOT have the trigger field populated as it has not come from a 'trigger condition'.
