Adding an Activity Log in Service Catalog Item

Hi All,
I am currently creating a Service Catalog Item for Contractual's Onboarding Process. The requirement is to see the activities in the form after it was submitted.
Let say they like to see the activities in each variables:
1. who changes or update the variable
2. when it was changed
3. the history how many times it was changed
4. the history of the value of each variable
The Audit History is enabled in sc_request and sc_request item but I still could not see the Activities (filtered) in the Form Layout.
Is it possible? Please advise.

Well............I did nothing !!!
Right Click on RITM form and do Personalize History :
1.Calender
2.List
- Mark as New
- Bookmark
- Subscribe
- Mute

Yes, but what I like to see is to have it in the RITM form itself and not only for ADMIN but for all ITIL users.
Similar to an Incident ticket.

HI,
Any ITIL user can see this kind of variables updates(image shared by Bhavesh) as an activity on RITM form by clicking on History<Calendar.

Hi Bhavesh,
How would I make these variables visible to ITIL users on the History-> List. I can see that these variables are already visible in History->calendar. My requirement is to show it in History-> List for ITIL users.
