I would like to add a title in the header of a widget. When I add this in the HTML template, it wraps around a second border to the widget. What is wrong here? I do not have the option to simply add a title like in a simple list widget.

You are on the Widget (the out-of-the-box one) on the last screenshot - you can't edit this one.
What you can do is edit the Title on Widget Instance.
In your case that would be Instance 2 - right above it. Just enter a Title and it'll appear on the page, you don't even have to wrap your second div around it.
You can also just navigate to your page, do a Ctrl + Right-click on the Most Viewed Widget Instance (in my that's a cloned widget from KB Most Viewed).
Go to Instance Options and change the title - voila, changed.
What I'm not sure of is, if you want an additional title or if we were just talking about the Title within the Bootstrap Panel.

Have you tried defining the title when you place the widget on the page? Here's a screenshot of the Requests Page with My Requests as the title.
