Catalog Task not created after approval of RITM

I had seen some weird issue on different catalog items where the approver is approving the RITM but the Catalog Tasks were not getting created. When we opened the workflow it was showing as pending for approval but in the approver tab the RITM is approved.

Can you share the details of your workflow? Screenshots of the workflow are appreciated. Also helpful is the properties page of the workflow.

I noticed you are using an approval coordinator around a single approval. That shouldn't be necessary in this case. Why not just use the user approval by itself before moving to WIP and creating your task?

Hi Chuck,
One weird thing here is that we couldn't replicate the issue. How can we think that there would be an issue with 'Approval Coordinator'? Can you please explain briefly? The issue is not only with particular catalog item or particular approve. We had seen for 3+ Catalog items with same issue and with different approvers had approved it.
