Hi,
To troubleshoot, can you change RITM view to default (currently on catalog item view), and scroll down to the bottom left and look for related links. There you can see the "Show Workflow" link, click that and see if the workflow matches what you've shown us.
This will tell you what happened during it. If this workflow wasn't published at the time this was ordered, and someone other than yourself ordered this, it won't use this workflow unless it was published (it'll use an older version or a completely separate flow).
Try ordering again if everything is confirmed true.

When I go to RITM's Default view, I do not see the 'Show Workflow' link.
But when I go the REQ associated with this RITM, I can see the Show workflow link.
When I click 'Show Workflow', this is what I see:
I have also ordered several times after the workflow was published. But all of these are not creating the Catalog tasks.

Hi,
So you have edited the out of box request workflow and so the RITM workflow has not kicked off yet.
There are two tasks now made for the request that need to be handled first, then it appears a third...then for the third you don't have the line drawn to the "end" activity. Please correct the request workflow first.

I have changed the OOB workflow - Service Catalog Request and Laptop Request to this:
But still in RITM no option to Show workflow.
I have checked in UI actions. Nothing has been changed to the OOB Show Workflow UI action.
