Change values for INC Urgency and Impact

I want to change the priority lookup for Incidents.
I know how to do this on the Priority Data Lookup table/form.
The issue is I need to add values to Impact and Urgency; and for some reason I am not able to do this.
When I go to the form to modify the Impact & Urgency combination, the dropdown is showing me choices from the INC Category field! (see attached).
When I go to dictionary to try and fix this in the choice list the fields for Impact or Urgency do not show up.
How can I create this matrix? Is this broken OOB? This is a brand new (untouched) installation.

These choices are automatically pulled from the choice values in the incident impact, urgency, and priority fields. If you want to add additional choices, you need to right-click the field labels on the incident form and select 'Configure choices'.
In my system, the dictionary entries for the choice lookup fields (on the lookup table itself) aren't pointing to the correct choice field. You'll also want to verify that these fields point to the matching field on the incident table to pull their choice value.

Thanks but for some reason When I go to dictionary to try and fix this in the choice list the fields for Impact or Urgency do not show up.
I resolved this by pulling some data from QA.
