Do we know if an incident is created based on a template

How can i know if an incident was created based on a template and whitch template was used ?

Hi,

You need to check the incident templates by navigating to System Definition -> Templates and filter by table incident. There you can see all the available incident templates.

From there, you can check based on the values that are being set, you will know which template is being used for the creation of the incident. The incident table as such does not store any information about the template.
