Has anyone had form sections disappear after adding 'annotation'?

I was recently editing a configuration item form when the form header disappeared...

Originally you could see two sections when you went to the form, one was the header for the form, which included a few key fields, and then another section called 'configuration'.

 

But when I went to personalize the form, the only section listed was called 'label', and it didn't show any fields (See attachment).

 

I started poking around and added '* Annotation' to the 'label' section. When I did so, the top section of the form disappeared and the 'configuration' section was all that was visible on the form.

 

I went back to the 'personalize form' slushbucket and removed '* Annotation' from the 'Selected' list, but when I saved, the top section   was still gone.

 

I've tried importing the record from other instances which haven't been changed, but that didn't help. I tried backing out the update set that was open while I was working on this, and that didn't help either.

We have also found that after one upgrade or another, there are sometimes multiple instances of the same form section on a form. The display *usually* shows the newest one, but when you edit, there seems to be some randomness to which one gets edited, making the behavior very unpredictable. Our habit: try to identify the original one and delete any others. Even then, you may have to repeat this exercise when you move the update set to another instance.

I currently have this exact same issue. I can restore to previous version, but as soon as I make any form layout changes, the form sections disappear. This problem seemed to start when I created a new form annotation type and added a new annotation to the form. Previous replies posted here did not help.
